What is Service Coordination?

The Service Coordinator at Lansdowne Children's Centre assists families with specialized needs, aiding them in accessing and coordinating services and resources, assisting with advocacy, and assisting with transitioning to other services.

Who is eligible for Service Coordination?

Lansdowne Children's Centre clients, who are involved with a variety of Lansdowne services but require additional assistance and coordination navigating the service system, may be eligible for involvement with Service Coordination. Referrals to the program are initiated by Lansdowne service providers that have involvement with the clients.

What can I expect?

Once a referral to Service Coordination has been completed you will be contacted by the Service Coordinator. Frequency of intervention will be determined by the current needs of your child and your family and the amount of involvement will vary based upon these needs. The Service Coordinator will provide assistance with access to other services and resources, advocacy issues, goal setting and planning, as well as case management. Service Coordination is actively involved in transition and discharge planning, particularly for the transfer to adult services.
