Job Posting: Family Service Coordinator

Family Service Coordinator .8 FTE – 12 to 18 month contract position

Responsibilities: As a Family Service Coordinator you will:

· Provide case management and coordination of services for children & youth and their families, up to age 21.

· Provide transition planning (school entry, high school, adult services with a focus on planning for adult services with transitional aged youth

· Advocate and plan for the changing needs of clients and families

· Lead collaborative efforts to problem solve and plan with community service system in an integrated approach to service planning and delivery

· Participate as an active member of Lifespan Team.

Qualifications: Preferred: Bachelor’s Degree in social work or a human service field

Minimum: Ontario College Diploma in a related field

Valid driver’s license and personal transportation

Experience: Preferred: Extensive experience of at least 3 years working with children and youth including responsibility for service coordination

Minimum: Related experience working with children and youth and/or responsibility for service coordination

Other: Confirmation of full COVID-19 vaccination (2 doses)

Salary Range: $27.07 to $31.84 per hour
