Anyone requiring information about Lansdowne Children's Centre services, wanting to make a referral to our services, or wishing to inquire if a child/youth is eligible for Lansdowne services, may contact SmartStart Hub. Our online referral form collects your contact and concern details. Referrals are processed by priority. Online referral forms are securely, electronically submitted to Intake Services at Lansdowne Children's Centre. Upon submission of the referral form an automated confirmation email is sent to the address of who is submitting this referral form, along with a copy of the form submission. An automated email notification is also sent, to the address of the noted Primary Contact alerting them to the form submission and the resulting planned phone call with SmartStart Hub. Only the Primary Contact will be contacted about the reasons for referral. The Primary Contact must be a legal guardian of the child for whom the referral is submitted. The Primary Contact can expect to receive a phone call from a SmartStart Hub team member within 90 days of the submission referral (approximately). This conversation with SmartStart Hub provides opportunity to discuss any concerns in more detail, and any services or supports that the child may be eligible to receive. If Lansdowne Children's Centre does not provide a requested service, our team will inform you of the appropriate community agency for the service.
