This media can include a full-length written feature story, a video interview, a series of short videos, a mix of photos, and/or any other creative applications to best illustrate your story and achieve the goals of the ambassador campaign. With Your Consent This Information is Shared Only with those professionals and organizations serving your child and family Through verbal and written communication, and in an electronic record The electronic health record lets Lansdowne Children’s Centre staff working with your child and family view and write down information about your child in a secure and confidential manner. It also means that you don’t have to repeat information as often, and it lets your team build on past progress and experience. Community partners included in your consent are listed on the “Consent for Information Sharing and Collection of Personal Information” form. You can say if you do not want to share certain information or if you do not want to share with certain people. Your child’s electronic record is secure and protected. This web‐based shared electronic record uses the same technology that is used for online banking. In order for members of your child’s team to use your child’s electronic record, they must: Be authorized by Lansdowne to access your child’s password‐protected record Attend training on the electronic record and related privacy policies and guidelines Sign a confidentiality agreement Your consent will be documented in the shared electronic record. Welcome to Lansdowne Children's Centre. Fall prevention, wellness and communication are examples of information shared to keep all children safe. Start Here Guide: Your Child's Safety and Wellness Information about communication, medication, equipment, and fall prevention. Return Next Communication Please make sure that Lansdowne staff involved in your child’s services have important health information. Describe your concerns and observations. If you don’t understand some part of your child’s condition or treatment, please ask Lansdowne staff to explain why things are being done the way they are. If you still have concerns, please ask again. When you are unsure, please ask. Get to know your Lansdowne Children’s Centre Team. If you are unsure who one of your service providers is, or what their role is, please ask. Lansdowne staff are trained in First Aid and cardiopulmonary resuscitation (CPR). There is a First Aid Station in the building. Fire and emergency preparedness drills occur regularly. In the event of an emergency, please follow the directions of Lansdowne staff. Medication Please make sure that Lansdowne staff are aware of the name and any possible side effects of any medication that a child/youth is taking. Please directly inform each Lansdowne staff that you are involved with about any allergies. If medication is to be given to a child/youth by Lansdowne staff, they must follow the Medication policy (ref. EI 1.07.2 - SS 1.19.2 Administering Medication) and ensure medication is in it’s original, child‐proof, labeled containers. Equipment Please make sure that your child’s equipment is kept clean and in good working order. Ask your child’s Occupational therapist (OT) or Physiotherapist (PT) to check that your equipment is safe and in good working order. Use equipment only as directed. Fall Prevention On entering Lansdowne please check-in with reception (where appropriate) and wait in the lobby for your service provider. Please do not wander the buidling without Lansdowne staff. Please always watch your children in the Lansdowne waiting room and/or therapy areas. Please help keep hallways free of clutter and obstacles. Report any tripping hazards both inside and outside the building. Please make sure your children/youth are wearing appropriate footwear, and be cautious of slippery floors during wet weather, including rain and snowfall. Caregiver-Mediated Early Years programs are no-cost to the family, part of the government funded Foundational Family Services. These play-based and child-led programs for young children, ages 12 months to 48 months, aim to build caregivers’ capacity to support their child’s skill development by teaching strategies that may help their young children learn new skills and meet individual goals. Welcome to Autism Services at Lansdowne Children's Centre in Brantford Foundational Family Services Caregiver-Mediated Early Years Program What is the Caregiver-Mediated Early Years Program? Autism Services Caregiver-Mediated Early Years programs are no-cost to the family, part of the government funded Foundational Family Services. These play-based and child-led programs for young children, ages 12 months to 48 months, aim to build caregivers’ capacity to support their child’s skill development by teaching strategies that may help their young children learn new skills and meet individual goals. Who is eligible? Your child must be a registered member of the Ontario Autism Program (OAP). Eligible families will receive an invitation from the Ministry via email and/or post mail. Autism Services at Lansdowne Children's Centre offers two service option models: Social ABCs and Pivotal Response Training (PRT). Checkout upcoming Workshops, online and in-person Event Calendar Social ABCs This is an early intervention program in which parents learn a set of strategies for helping their Autism Spectrum Disorder (ASD) diagnosed toddler communicate and interact socially, using play-based activities and daily routines that their child finds most motivating. Pivotal Response Training (PRT) A variation of Applied Behavioral Analysis (ABA) type therapy, Pivotal Response Training focuses on more comprehensive “pivotal” areas, such as increasing a child's motivation to learn, initiate communication, and monitor their own behaviors. Early Social Interaction/ Social Communication Emotional Regulation and Transactional Supports (ESI/SCERTS) The ESI/SCERTS model is a research-based educational approach and multidisciplinary framework that directly addresses the core challenges faced by children and persons with Autism Spectrum Disorder (ASD) and related disabilities, and their families. I am a 12 year old loving with Spinal Muscular Atrophy (SMA), a motor neuron disease that affects the voluntary muscles in my body. My condition causes me to have weakness throughout my body and confines me to a power wheelchair to get around. Make no mistake though – I live my life to the fullest! I love spending time with my friends and siblings, love school, camping, baking and so much more. I really love the recreation programs at Lansdowne and have made some great friendships with the other children and staff. This is a place free of judgment that helps me explore my independence and creativity. We are thankful for the programming at the Centre!" - Zo(e) Through these programs, Zoe is able to see that she is not alone. She is free from judgment, and she feels valued, and a sense of belonging. She has made friendships with other children (and even the staff!), explored her creativity and independence, learned new things, and most of all, had so much fun and made so many memories. Along with the rec programs, Zoe also accesses physiotherapy, occupational therapy, and social workers, all of which has helped her tremendously. They have helped our family navigate through endless paperwork to get Zoe all of the equipment and support she needs. "We truly don't know what we do without the help of Lansdowne.
