Anyone with a concern about their child's development can contact SmartStart Hub to start the referral process.
No diagnosis or doctor referral is required. SmartStart Hub at Lansdowne Children's Centre is your one-stop solution for connecting with services and supports for communication, developmental and physical concerns in the Grand Erie district.
There is NO COST to the family for accessing SmartStart Hub services, supports and resources.
Interactive checklists provide a guide of development milestones and resources to help you measure and encourage development progress.
If you have multiple incomplete milestones, and especially if you have incomplete milestones from an earlier age group, please consider completing a referral with SmartStart Hub to access opportunities for assessment, support and therapy services.
Anyone requiring information about Lansdowne Children's Centre services, wanting to make a referral to our services, or wishing to inquire if a child/youth is eligible for Lansdowne services, may contact SmartStart Hub. Our online referral form collects your contact and concern details. Referrals are processed by priority. Online referral forms are securely, electronically submitted to Intake Services at Lansdowne Children's Centre. Upon submission of the referral form an automated confirmation email is sent to the address of who is submitting this referral form, along with a copy of the form submission. An automated email notification is also sent, to the address of the noted Primary Contact alerting them to the form submission and the resulting planned phone call with SmartStart Hub. Only the Primary Contact will be contacted about the reasons for referral. The Primary Contact must be a legal guardian of the child for whom the referral is submitted. The Primary Contact can expect to receive a phone call from a SmartStart Hub team member within 90 days of the submission referral (approximately). This conversation with SmartStart Hub provides opportunity to discuss any concerns in more detail, and any services or supports that the child may be eligible to receive. If Lansdowne Children's Centre does not provide a requested service, our team will inform you of the appropriate community agency for the service.
