They are knowledgeable about services and funding resources that your family may benefit from. Family Service Coordinators help families understand what services they have, what services they may need and when services may change (due to child's age or eligibility). They provide advocacy for child/youth needs in education, health and the community. Eligibility Start Referral Family Service Coordinators may be referred to as your child’s "Case Manager," who encourage and support all services to work together as a team. They work with clients and their families living in Brantford, Brant County, Six Nations of the Grand River and Mississaugas of the Credit First Nation. Join this police-escorted, no touch-down motorcycle ride leaving from Brantford to tour Grand River communities and scenery, including Haldimand, Norfolk and Brant counties. Your registration is ALL INCLUSIVE of the day's entertainment, including: Live Music, Vendor Displays, Continental Breakfast, Hot Lunch and Door Prizes! The 90-minute, 107-kilometre police-escorted ride travels east along Brant County Hwy 54 to Haldimand County/Caledonia, and south along Hwy 6 to Hagersville, turning west onto Indian Line Road and returning to Brant County through Oakland, Scotland and north on Hwy 24, taking Hwy 403 to Oak Park Rd (Exit 30) to return to Brimich Logistics. Brantford/Brant County - Haldimand/Norfolk County - Mississaugas of the Credit First Nation - Six Nations of the Grand River - Other Region of Ontario AUTISM SERVICES All Services Index AUTISM SERVICES Intensive ABA Therapy, Birth to 12 years old What is Intensive ABA therapy? Intensive ABA therapy treatment is 6+ hours and/or a minimum of 3 scheduled appointments per week. Treatment is primarily provided at a 1:1 - child to therapist - ratio, with daily opportunities for interaction and play with peers. Children will be taught essential skills and the management of challenging behaviours, working across several developmental domains, including: • Language • Communication • Play • Social interactions • Personal hygiene • Toileting • School-readiness Who is eligible? Intensive ABA therapy is available for children/youth, age birth to 12 years, with an Autism Spectrum Disorder (ASD) diagnosis. A screening process will be applied to confirm eligibility for this program, including registration with the Ontario Autism Program (OAP). Eligibility Start Referral Intensive ABA is available for children diagnosed with Autism Spectrum Disorder, up to age 6 years old. Registration with OAP is not required but is recommended. Goals, Skills and Keywords Expectations •Individualized assessment will determine specific goals for your child before treatment begins. •There will be an emphasis upon parent involvement and training within this intervention. •Therapy will be provided at Lansdowne Children’s Centre in Brantford. •Intensive ABA therapy is a commitment of 6+ hours per week. Welcome to Lansdowne Children's Centre. The importance of attending appointments, consistently and on time. Start Here Guide: Client Appointment Attendance If you are unable to keep an appointment please contact the Schedulers Office at least 24 hours in advance, by phone at 519-753-3153 or by email booking@lansdownecc.com to let us know you are cancelling. Return Next Attendance If you need to cancel an appointment with less than 24 hours' notice, please contact your therapist directly. The aim of our attendance policy is not meant to be a burden. It helps us to offer a high standard of service to our clients, and to move clients from our waitlist to services as quickly as possible. Your child’s goals will only be able to be met when you come regularly to all appointments. If something is getting in the way of your child attending therapy appointments, please speak with your child’s therapist as soon as possible to make a new plan. Missed Appointments In order to ensure the most effective utilization of costly and limited resources, Lansdowne Children’s Centre clients are expected to attend appointments as scheduled jointly by clients/caregivers, schedulers and clinicians. In the event of an identified pattern of regular nonattendance, Lansdowne reserves the right, after careful consideration, including (attempted) discussion with the client family/caregiver(s), to discontinue or discharge a client from services. No show visits or late cancellations (i.e. within 24 hours) that occur twice consecutively or that add up to one-third of an agreed upon block of treatment, shall be considered an identified pattern. Clinicians (in consultation with program manager as applicable), shall exercise discretion and professional judgment when discontinuing/ discharging a client for an identified pattern of regular nonattendance and shall consider extenuating circumstances or those beyond the caregiver’s control. Site Closures Lansdowne Children’s Centre is committed to maintaining service delivery to clients at all times. Building closures should be rare and only occur in extreme circumstances, for example, when weather conditions threaten the travel, safety and/or wellbeing of Lansdowne staff and clients. Closures are determined on a site by site basis (i.e. Brant, Haldimand or Norfolk locations). Lansdowne will communicate closures as soon as possible based on information available at the time. Notice of site closures will be posted on our website and social media channels (i.e. Facebook, Instagram), and shared with local radio partners (CKPC 92.1 FM and 98.9 myFM). The Lansdowne automated voice message greeting will also be updated for when clients call reception. Please note that Lansdowne will not necessarily close in the event that school buses are cancelled and/or in the event that a particular school closes in the area. What is the Lansdowne Ambassador Program? A Lansdowne Ambassador typically engages with our communications team to create media in support of telling a Lansdowne that is shared as part of a promotional and/or advocacy effort for an event, fundraising or awareness campaign. This media can include a full-length written feature story, a video interview, a series of short videos, a mix of photos, and/or any other creative applications to best illustrate your story and achieve the goals of the ambassador campaign. To express your interest and learn more about telling your story with Lansdowne, please tell your Lansdowne service provider, or contact Communications Supervisor Joshua Paxton by email, jpaxton@lansdownecc.com What Clients Need to Know About Information Sharing In order to develop a care plan and provide your child and family with the services that best meet your needs, staff at Lansdowne Children’s Centre will ask for information about your child and family. With Your Consent This Information is Shared Only with those professionals and organizations serving your child and family Through verbal and written communication, and in an electronic record The electronic health record lets Lansdowne Children’s Centre staff working with your child and family view and write down information about your child in a secure and confidential manner. It also means that you don’t have to repeat information as often, and it lets your team build on past progress and experience. Community partners included in your consent are listed on the “Consent for Information Sharing and Collection of Personal Information” form. You can say if you do not want to share certain information or if you do not want to share with certain people. Your child’s electronic record is secure and protected. This web‐based shared electronic record uses the same technology that is used for online banking. In order for members of your child’s team to use your child’s electronic record, they must: Be authorized by Lansdowne to access your child’s password‐protected record Attend training on the electronic record and related privacy policies and guidelines Sign a confidentiality agreement Your consent will be documented in the shared electronic record. Caregiver-Mediated Early Years programs Autism Services Caregiver-Mediated Early Years programs are no-cost to the family. These play-based and child-led programs for young children, ages 12 months to 48 months, aim to build caregivers’ capacity to support their child’s skill development by teaching strategies that may help their young children learn new skills and meet individual goals. Streaming Video Resources The Autism Services Brant webinar gallery is an online video streaming resource that you can access at no-charge. This collection of helpful videos offer an introduction to autism, tools for identifying behaviour, tips for improving your child's quality of life and ideas for activities you can try at home with your child. What can I expect? To access each video you will be asked to register your email contact information and OAP registration number. This information is held privately and only used by Autism Services for the purpose of stat-tracking for budget requirements, and to follow up with you on your streaming video experience. Videos are hosted on YouTube. Part of our FREE Foundational Family Services , you can stream webinar recordings at your convenience. Brantford/Brant County - Haldimand/Norfolk County - Mississaugas of the Credit First Nation - Six Nations of the Grand River FAMILY ENGAGEMENT All Services Index FAMILY ENGAGEMENT Family Advisory Council (FAC) The Family Advisory Council’s role is to advocate for client and family centered care at Lansdowne Children Centre. The F.A.C. will advise and make recommendations in order to promote quality programs and services within the Centre. Engaging families to help improve the quality of service with ongoing communication, collaboration and partnership between families and Lansdowne Children’s Centre. Responsibilities: • Provide valuable input from a family perspective as they relate to LCC programs and services. • Enhance positive staff - client relations • To assist in identifying the needs and priorities of clients/families through team work with the LCC staff • Actively assist to implement change • Assist with the development and access to information, support and educational resources which help our clients/families Eligibility Apply to Join Membership: The Council will consist of a minimum of 6 members including a)Parent/family representatives* b)Family Engagement Coordinator – a LCC staff member *Family representatives will consider diversity in gender, age, diagnosis and geography to enable a broad representation of background and experiences. Recruitment: •Members will be recruited annually via Lansdowne Children’s Centre’s social media channels •Prospective new members will complete an application form •The Family Engagement Coordinator will select candidates upon form completion Qualifications: •Have a general understanding of the broad range of clients and families we serve •Respects differing opinions to be inclusive to the full diversity of family experiences; socio-economic status, language, race, color, religion and identity. •Works collaboratively with staff, clients and families •Maintain a sense of humor, a sense of vision and a positive attitude •Maintain respect for privacy and confidentiality •Available to attend at least 75% of the quarterly meetings via in person and/or zoom •Members will sign a volunteer form, a privacy and confidentiality form and obtain a Vulnerable Persons Check to participate •Immunization: In order to encourage participation of family members across our catchment region of Grand Erie (including Brantford, Counties of Brant, Haldimand and Norfolk, Six Nations of the Grand River, and Mississaugas of the Credit First Nations) the Family Advisory Council (FAC) will meet using a hybrid (in-person & virtual) model. Participants attending in-person must provide record of receiving at least two COVID-19 vaccines.” Virtual attendance will be supported without disclosure of vaccination. Goals, Skills and Keywords Expectations Logistics: •The FAC will meet 4- 6 time a year during September to June •The Family Engagement Coordinator will make agendas, take minutes and provide support during meetings •Communications ( agendas and minutes) will be emailed to members on a regular basis •The groups will communicate primarily through emails between meetings as needed •Lansdowne is committed to supporting member’s ability to attend meetings, providing support as needed to remove barriers to participation. •The term of office for the Family Advisory Council shall be two years with an opportunity to extend membership. Roles: Chair •Call and chair meetings •Develop meeting agenda with Family Engagement Coordinator •Review meeting minutes •Facilitate open communication, discussions and participation during meetings Decision Making: •The group will aim for consensus in decision making whenever possible. Where there is no consensus a majority will be accepted. •All voting members will have a vote •A minimum of 50 per cent of members must be present for a vote to be taken •The Family Engagement Coordinator will not vote unless a tie is needed to broken • Reporting Relationship: : The Family Engagement Coordinator will function as the liaison between the Council and staff. The Family Advisory Council exchanges information with the Centre through the Family Engagement Coordinator as the group’s representative. You Belong Here.
