This early years development checklist provides physical and communication development milestones that most children are demonstrating in each age group. Check off the abilities that your child is demonstrating on a consistent basis. If you have concerns about unchecked milestones. first consult the online resources provided for more information. The recommended streaming videos and printable activities will help you enage in play-based learning with your child. Often these universal strategies for skill development will help a child's progress. If you have multiple incomplete milestones, and especially if you have incomplete milestones from an earlier age group, please consider completing a referral with SmartStart Hub to access opportunities for assessment, support and therapy services. To be eligible to register for the Ontario Autism Program, your child must: Be under age 18 Currently live in Ontario Have a written diagnosis of Autism Spectrum Disorder from a qualified professional Your child’s written diagnosis must include: Your child’s full name and date of birth The date of your child’s assessment A statement indicating that the child meets the diagnostic criteria for autism spectrum disorder The qualified professional’s name and credentials First Steps for Families Are you looking for help with registration paperwork? Call our Autism Services Family Support Worker at 519-753-3153 ext. 268 #3 Are you registered for the Ontario Autism Program? Register for OAP You must fill out and submit the Ontario Autism Program registration form, provided by the Ontario Ministry of Children, Community and Social Services (the Ministry). Your child or youth must meet the program eligibility criteria to be registered. Contact the Ministry’s central intake and registration team (CIRT) by emailing oap@ontario.ca or calling 1-888-444-4530. You do not need to register again if you have previously registered your child for the Ontario Autism Program. Your child’s registration date is based on when the Ministry receives your registration form and all supporting documents. The registration list is controlled and monitored by the Ministry and Lansdowne staff is not able to access this information. #4 Are you registered with Lansdowne Autism Services? If you are interested in Autism services at Lansdowne or purchasing individual treatment, you will need to register directly with Lansdowne Autism Services. This list is internally controlled and we can let you know if you are currently on our list to purchase treatment. Registration for the Ontario Autism Program (OAP) or other Lansdowne services (i.e. Speech), does not automatically register you with Autism Services. Please complete a referral form with our SmartStart Hub team to get connected with Autism Services. You can complete the online form, OR call our team at 519.753.3153 x507. Complete a Referral Form Register for Fee-for-Service Childhood Budgets/ One Time Funding Childhood budgets provide money directly to you to purchase the services and supports that are most important to you and your child. CLICK HERE for a list of eligible expenses. including purchasing behavioural services, as well as other areas where you can spend your funding has been compiled. Families with children under age six are eligible to receive $20,000 per child. Families with children aged six and older are eligible to receive $5,000 per child. Families who accept a childhood budget can be assured that their child’s position on the provincial waitlist will be maintained for entry into the new needs-based, sustainable Ontario Autism Program. Every gift is precious. Two unique donors are Ken and Tammy Breau. They have a unique perspective and a personal connection to children with special needs. Ken and Tammy own three Dairy Queens in our community. They recently began the Red Shed initiative that allows charities in our community to use the shed to fund raise. Ken and Tammy have always supported our Lansdowne foundation. Thank you for wonderful gifts and we always enjoy the treats at DQ. In Kens own words he tells us why Lansdowne is so important to the couple. I am a 12 year old loving with Spinal Muscular Atrophy (SMA), a motor neuron disease that affects the voluntary muscles in my body. My condition causes me to have weakness throughout my body and confines me to a power wheelchair to get around. Make no mistake though – I live my life to the fullest! I love spending time with my friends and siblings, love school, camping, baking and so much more. I really love the recreation programs at Lansdowne and have made some great friendships with the other children and staff. This is a place free of judgment that helps me explore my independence and creativity. We are thankful for the programming at the Centre!" - Zo(e) Through these programs, Zoe is able to see that she is not alone. She is free from judgment, and she feels valued, and a sense of belonging. She has made friendships with other children (and even the staff!), explored her creativity and independence, learned new things, and most of all, had so much fun and made so many memories. Along with the rec programs, Zoe also accesses physiotherapy, occupational therapy, and social workers, all of which has helped her tremendously. They have helped our family navigate through endless paperwork to get Zoe all of the equipment and support she needs. "We truly don't know what we do without the help of Lansdowne. Lansdowne Children's Centre has a rich history in Brantford and the surrounding area. As the Grand Erie region's SmartStart Hub, Lansdowne connects families with top quality services, supports and resources, for children and youth with special needs, and their families. What is Lansdowne? Lansdowne Children’s Centre is one of Ontario’s 21 Children’s Treatment Centres [CTCs] and a member of Empowered Kids Ontario (EKO). Lansdowne provides services and supports for over 3,000 children and youth annually, and their families. Lansdowne provides child development resources, supports and services for communication, developmental and physical needs. Our service region includes Brantford, Brant, Haldimand and Norfolk counties, and the Six Nations of the Grand River and Mississaugas of the Credit First Nation. Lansdowne Service Guide Annual Reports and Board Info SmartStart Hub, Screening Tools Speech & Language @ Lansdowne Physiotherapy @ Lansdowne Therapeutic Recreation (TR) @ Lansdowne Speech & Language @ Lansdowne 1/9 Our Story The origins of Lansdowne Children's Centre go back to 1952 in Brantford, Ontario, when a group of seven families whose children lived with cerebral palsy met on a part-time basis in the Red Cross Rooms on William Street, forming the Parents Council for Cerebral Palsy. Over the next decade this group would evolve to become the Brant County Cerebral Palsy Association, partnering with the Rotary Club of Brantford in 1960 to open the Crippled Children/Cerebral Palsy Centre at the Brantford General Hospital. In 1973, the Centre moved to the Clinical Division and Educational Program location at Preston Boulevard, housed in one wing of Lansdowne School. In 1974, the Lansdowne Children's Centre became incorporated and was recognized as a Group K Hospital under the Ministry of Health, offering outpatient services in physical medicine therapies. The Centre began receiving funding from the Ministry of Health and the Cerebral Palsy Association altered its role to one of financial support. Infant Development Program and Special Services at Home Program transferred their administration to Lansdowne Children's Centre in 1985, and became an integral part of our services. Our services expanded again in 1987, adding a Respite Care Program and School Health Support Services Program, and again in 1991 with the addition of the Early Integration Program. In 1997 the Ontario Ministry of Health named Lansdowne the lead agency for the Brant Preschool Speech and Language Program. In 1998, on June 25th, Lansdowne Children's Centre celebrated the Grand Opening of a new building at 39 Mount Pleasant Street in Brantford. 1999 marked Lansdowne's 25th Anniversary as a Children's Treatment Centre (CTC). 1998 Planning for Lansdowne Children's Centre, Brantford 1998 Construction of Lansdowne Children's Centre, Brantford From www.jjackowetz.com the original Jane Laycock School that later became Lansdowne Children's Cent 1998 Planning for Lansdowne Children's Centre, Brantford 1/5 On July 7th in 2000, Lansdowne celebrated the unveiling and dedication of the Joanne Renahan Children's Courtyard. Joanne, her son Stephan and husband Peter were in attendance to mark the event. In 2002 the Kid’s Country Inn was opened, introducing a weekend respite program for medically fragile and/or technologically dependent children and youth. In June 2004, the Early Autism Program was transferred to Lansdowne Children’s Centre, helping children to develop their skills in the areas of play, communication, socialization and self-help In 2007, special flexible respite care services were started for families caring for children or youth diagnosed with Autism Spectrum Disorder (ASD). In 2014 Lansdowne introduced Holistic Integrated Teams [HITs] to help centralize family communication and coordinate services and supports for clients with multiple service needs. In 2019 Lansdowne expanded its regional coverage after assuming management of service provider contracts for Haldimand and Norfolk School Based Rehab Services (2020). In 2020 Haldimand-Norfolk Preschool Speech & Language was also added, creating a unified Brant- Haldimand-Norfolk program managed by Lansdowne Children's Centre. Also in 2020, Lansdowne assumed operation of the Alternate Care family respite program. Over 20 years removed from the grand opening celebration, the expansion of services at Lansdowne and growth of the service and support teams have generated concerns about the property's capacity. In 2020, amidst a global pandemic, Lansdowne Children's Centre quickly 'adapted' service delivery to a virtual model to accommodate children and youth clients and their families. Combining virtual and in-person care as a hybrid model allowed Lansdowne to stay connected and serve families. The virtual option also helps to address main-site crowding and parking/accessibility concerns, and allowed Lansdowne to deliver service to over 3,000 children and youth and their families. In 2021, Lansdowne celebrated the dedication of new service locations in Haldimand and Norfolk counties. The additional locations are opened in Caledonia, Simcoe and Dunnville, improving Haldimand-Norfolk access to rehabilitation services. 20th anniversary Lansdowne Charity Motorcycle Ride #ForTheKids, Saturday July 6. Join this police-escorted, no touch-down motorcycle ride leaving from Brantford to tour Grand River communities and scenery, including Haldimand, Norfolk and Brant counties. Your registration is ALL INCLUSIVE of the day's entertainment, including: Live Music, Vendor Displays, Continental Breakfast, Hot Lunch and Door Prizes! Subject to change. RIDE ROUTE The Charity Motorcycle Ride for Lansdowne Kids will leave from Brimich Logistics along Hardy Road, riding to Brant Ave and Colborne St. West before turning left onto Mt. Pleasant Street to ride past Lansdowne Children's Centre (approx. 11:10 am). The 90-minute, 107-kilometre police-escorted ride travels east along Brant County Hwy 54 to Haldimand County/Caledonia, and south along Hwy 6 to Hagersville, turning west onto Indian Line Road and returning to Brant County through Oakland, Scotland and north on Hwy 24, taking Hwy 403 to Oak Park Rd (Exit 30) to return to Brimich Logistics. Below is a description of our registraiton and fundraising platform experience, that includes a personalized fundraising webpage and link you can share with friends and family. STEP 1) Registration TAKE ADVANTAGE OF EARLY BIRD PRICING! REGISTER TO RIDE BEFORE APRIL 30-2025 Choose to register your rider(s) or fundraiser(s) by completing the checkout process. Select the number of riders and rider + passengers you're going to signup today OR, choose Fundraising Only (No Cost) to collect pledges for the campaign but not attend the ride event. When your form is complete, click the green button at the bottom of the page, SELECT STEP 2) Payment Portal This is the start of the payment portal. Complete all the fields to proceed. You will be asked for the buyer information (which must match your payment card). As well, each registration ticket will ask you for the names and contact details of your riders and passengers. Please complete these details so we can contact you with event details. Just above your payment card details is a drop down menu to make a contribution to ZEFFY, the organization that hosts this registration platform. We are not charged to use this online service AND do not get charged processing fees on your credit or debit card payments. This service (ZEFFY) is supported by donations made during the checkout process. If you choose not to donate in support of this online service you can use the drop down menu to select 'Other' and enter $0.00. Your choice to support or decline donations to Zeffy does not impact the amount of monies pledged or raised by your participation. Now that you're riding for Lansdowne Kids, your profile page will ask if you would like to register your fundraising as an Individual or as part of a Team. Your registration ticket fee(s) are the first funds raised in your pledge efforts. As an Individual you will get your own fundraising page and goal thermometer to tell your story and collect pledges from your friends and family. As a Team, your registration and fundraising efforts will be combined with other team members in a shared goal thermometer and team page. STEP 4) - Customize your profile page Your campaign was successfully created! Now you can choose to Edit Your Profile and Form, or see your Form (filled with our default content) and start sharing your link. On your pledge page you can accept donations to support your fundraising, and sign up additional riders to join the Charity Motorcycle Ride for Lansdowne Kids. WHY: Proceeds raised by the Charity Motorcycle Ride presented by Ensuring Brant are donated to Lansdowne Children's Centre Foundation . These funds are invested into programs and services accessed by infants, children and teens with physical, communication and/or developmental additional/special needs.
