Brantford/Brant County - Haldimand/Norfolk County - Mississaugas of the Credit First Nation - Six Nations of the Grand River LIFESPAN SUPPORT SERVICES All Services Index LIFESPAN SUPPORT SERVICES Family Service Coordinator (FSC) Family Service Coordinators support families of children and youth with multiple and/or complex special needs. They are knowledgeable about services and funding resources that your family may benefit from. Family Service Coordinators help families understand what services they have, what services they may need and when services may change (due to child's age or eligibility). They provide advocacy for child/youth needs in education, health and the community. Eligibility Start Referral Family Service Coordinators may be referred to as your child’s "Case Manager," who encourage and support all services to work together as a team. They work with clients and their families living in Brantford, Brant County, Six Nations of the Grand River and Mississaugas of the Credit First Nation. Goals, Skills and Keywords Expectations Family Service Coordinators help families develop goals, learn advocacy skills, and ensure that a family’s needs and concerns are heard and respected. Family Service Coordinators can host and/or attend meetings, medical appointments, therapy sessions and assessments. Family Service Coordinators: Help guide you through transitions and changes in school placements. Provide appropriate documentation through individual sessions or information clinics. Support the development of a coordinated service plan that ties goals and services together ensuring the unique needs of each family is documented and shared. Brantford/Brant County - Mississaugas of the Credit First Nation - Six Nations of the Grand River RECREATION SERVICES All Services Index RECREATION SERVICES Therapeutic Recreation (TR) Recreation Services provide opportunities designed to assist and encourage children and youth clients to take advantage of individual or group activities directed towards promoting growth and development. These activities allow clients to explore their interests, exercise their skill sets and pursue personal development. Eligibility Start Referral Therapeutic Recreation programs are designed for participants who: Are 10 – 18 years of age Are receiving services at Lansdowne Children’s Centre. Have a level of independence which allows safe supervision as well as maintaining self-directed participation at a 3 youth to 1 adult ratio Identify a personal interest in individual and/or TR program goals Goals, Skills and Keywords Expectations TR services focus on opportunities to: Strengthen skills necessary for participation Discover and build upon personal interests Increase social skills/opportunity Maximize independence Foster community involvement Following referral, the Recreation Therapist (RT) will complete a TR initial assessment with Guardian(s) to support choice of a program option that will offer the most benefit to the participant. Formal/informal assessments will be completed as appropriate to support program development as well as set and monitor goals. Programs are typically group sessions. 1:1 appointments are available after consultation. Group Programming Programs are in house or in the community dependent on the leisure pursuit During the school year programs are in evenings and occasional weekends During the summer there are 4 weeks of camp that are offered Individual Appointment 1:1 appointments with the RT to determine next steps in an individual pursuit Transition to community programming You Belong Here. You can also consult the Brant County Health Unit online (www.bchu.org ) or Health and Social Services Haldimand and Norfolk online (www.hnhu.org ) Return Next Guidelines Regarding Absence Due to Illness Lansdowne Children's Centre needs to provide a healthy setting. To help with this we ask that you please stay at home and away from the centre and staff when ill (based on Brant County Health Unit guidelines). Lansdowne reserves the right to send clients home from scheduled services and programs if our staff believe that the client is too ill to join in activities. Brant County Health Unit guidelines also apply when Lansdowne services are delivered in a client home or community-based venue. Please let Lansdowne staff know if you or anyone in your family/household is ill if a home visit is scheduled. Keeping your hands clean is the best way of preventing the spread of germs. Use the provided hand sanitizer upon entering the building. Please wash your hands after handling any type of soiled material or using the washroom. Lansdowne staff are committed to sanitizing their hands prior to and after working with children and youth. Feel free to ask them if they have done so. Therapy equipment and surfaces used are cleaned and maintained regularly to ensure everyone’s wellness and safety. Children and other family members with the illnesses listed below should stay at home and away from Lansdowne as noted: Chickenpox - until well enough to join in all activities and having no fever, whether or not there is a rash Diarrhea ‐ no diarrhea for 24 hours Fever ‐ no fever for 24 hours Head lice ‐ until after the first treatment and no nits are in the hair Impetigo ‐ until an antibiotic has been taken for at least 24 hours Influenza (flu) ‐ until 5 to 7 days after onset of symptoms Pink Eye, bacterial (conjunctivitis) ‐ until the antibiotic has been taken for at least 24 hours Ringworm ‐ until treatment has started Strep Throat ‐ until an antibiotic has been taken for 24 hours Vomiting ‐ no vomiting for 24 hours Return Next
