SS@H can be used in a variety of ways to best meet the needs of each unique family. Support can be provided either in the home or in a variety of community settings The program is funded by the Ministry of Children, Community and Social Services. Some of the ways SS@H can provide your child with support are: 1:1 Support Worker to support your child in working toward their own unique goals. 1:1 Support Worker to provide families with a short break from the demands of caring for their child. The purchase of a variety of other individualized supports and services for your child based on current Ministry guidelines. Brantford/Brant County AUTISM SERVICES All Services Index AUTISM SERVICES Entry to School The Entry to School program was introduced as a bridge to kindergarten for children diagnosed with Autism Spectrum Disorder (ASD). Families registered with OAP are selected and invited by the government agency to participate in the program, hosted by Autism Services at Lansdowne Children's Centre in Brantford. Eligibility Start Referral 1) Registered with the Ontario Autism Program (OAP) 2) Receive an invitation letter from the OAP/Provincial Ministry is required. After receiving your letter please call SmartStart Hub at 519.753.3153 x507 to connect with Autism Services. 2) Child is has not yet enrolled in school and is under the age for grade one. Goals, Skills and Keywords Expectations Participants begin on their own, meeting the classroom as a lone individual free to explore as they interact with adults. When they return for group play, they are one of four in the classroom, with groups alternating days. As the program progresses the groups come together to form a group of eight. Care providers are screened and trained to provide relief either in their home, the family’s home or in the community. It is a safe and nurturing place for your child to spend time while providing a much needed break for the caregivers. The Alternate Care Program is licensed under the Ministry of Children Community and Social Services and administered by Lansdowne Children’s Centre.Your child will have an enjoyable time with another family while you are taking care of yourself. Eligibility Start Referral Children/youth birth to 18 years of age with a diagnosis of developmental delay. Goals, Skills and Keywords Expectations Initially the Alternate Care Supervisor will meet with you to obtain more information about your child’s individual needs and your respite requirements. Options for ACP providers will be discussed. Service initiation could begin sooner if you know of an individual or family that could provide support. The Social Workers of Lansdowne Children’s Centre are professionally trained and registered members of the Ontario College of Social Workers Social Service Workers. Social Workers can provide supportive counselling to help support mental wellness including assistance in developing positive coping strategies, problem solving overwhelming loss/grief difficulty coping with a child’s diagnosis or prognosis Eligibility Start Referral Children and youth receiving services from Lansdowne Children's Centre and their families are eligible for Social Work Services. Referrals can be requested by the family or youth or may be suggested by another Lansdowne service provider working with the child and their family. Goals, Skills and Keywords Expectations Once a referral to Social Work has been completed you will be contacted by a member of the Social Work team. Service needs are determined through goal-based care plans and may include individual or family intervention and support. Social Workers can provide supportive counselling to help with: Adjustment to illness & disability Client & caregiver stress & coping Connecting to financial & community resources Building confidence and self-advocacy skills Providing support to the whole family Various groups and programs may be offered for sibling support needs, youth transitions, healthy relationships & sexual education. The F.A.C. will advise and make recommendations in order to promote quality programs and services within the Centre. Engaging families to help improve the quality of service with ongoing communication, collaboration and partnership between families and Lansdowne Children’s Centre. Responsibilities: • Provide valuable input from a family perspective as they relate to LCC programs and services. • Enhance positive staff - client relations • To assist in identifying the needs and priorities of clients/families through team work with the LCC staff • Actively assist to implement change • Assist with the development and access to information, support and educational resources which help our clients/families Eligibility Apply to Join Membership: The Council will consist of a minimum of 6 members including a)Parent/family representatives* b)Family Engagement Coordinator – a LCC staff member *Family representatives will consider diversity in gender, age, diagnosis and geography to enable a broad representation of background and experiences. Recruitment: •Members will be recruited annually via Lansdowne Children’s Centre’s social media channels •Prospective new members will complete an application form •The Family Engagement Coordinator will select candidates upon form completion Qualifications: •Have a general understanding of the broad range of clients and families we serve •Respects differing opinions to be inclusive to the full diversity of family experiences; socio-economic status, language, race, color, religion and identity. •Works collaboratively with staff, clients and families •Maintain a sense of humor, a sense of vision and a positive attitude •Maintain respect for privacy and confidentiality •Available to attend at least 75% of the quarterly meetings via in person and/or zoom •Members will sign a volunteer form, a privacy and confidentiality form and obtain a Vulnerable Persons Check to participate •Immunization: In order to encourage participation of family members across our catchment region of Grand Erie (including Brantford, Counties of Brant, Haldimand and Norfolk, Six Nations of the Grand River, and Mississaugas of the Credit First Nations) the Family Advisory Council (FAC) will meet using a hybrid (in-person & virtual) model. Participants attending in-person must provide record of receiving at least two COVID-19 vaccines.” Virtual attendance will be supported without disclosure of vaccination. Goals, Skills and Keywords Expectations Logistics: •The FAC will meet 4- 6 time a year during September to June •The Family Engagement Coordinator will make agendas, take minutes and provide support during meetings •Communications ( agendas and minutes) will be emailed to members on a regular basis •The groups will communicate primarily through emails between meetings as needed •Lansdowne is committed to supporting member’s ability to attend meetings, providing support as needed to remove barriers to participation. •The term of office for the Family Advisory Council shall be two years with an opportunity to extend membership. Roles: Chair •Call and chair meetings •Develop meeting agenda with Family Engagement Coordinator •Review meeting minutes •Facilitate open communication, discussions and participation during meetings Decision Making: •The group will aim for consensus in decision making whenever possible. Where there is no consensus a majority will be accepted. •All voting members will have a vote •A minimum of 50 per cent of members must be present for a vote to be taken •The Family Engagement Coordinator will not vote unless a tie is needed to broken • Reporting Relationship: : The Family Engagement Coordinator will function as the liaison between the Council and staff.
